qSDS Procedure Template

Procedure documentation needs to be a clear, unambiguous 'manual' for accomplishing a given task or result. It must be distributed to - and understood by - everyone who executes the procedure, or depends upon it.

The following template can be used as the basis for procedural documentation:

Process <The process to which this procedure belongs>
Creation date <Date of creation>
Author <Author name>
Approved by <Quality / Process manager>
Approval date <Approval date>
Revisions <Dates and details of all revisions to the procedure since its creation>

Synopsis

<A synopsis of the procedure, stating its overall purpose, the context in which it is used, and the groups of people to whom it is relevant>

Triggers

<The circumstances or events that 'trigger' the procedure>

Inputs

<Optional or mandatory information, materials or outcomes from other procedures that are used by this procedure>

Roles

<Personnel roles to whom this procedure is applicable>

Steps

  1. <Step 1 of the procedure>
  2. <Step 2 of the procedure>

Output

<The tangible or intangible results expected from the procedure>

Evaluation

<The (optional) criteria by which the success of the procedure is evaluated>
